Answered to the best of my knowledge (others please contribute/correct):

1) All the stuff is actually there. There should be a mess in the uppper
right corner, but is not arranged correctly. You can use the Vgui Editor to
arrange them correctly (Ctrl-Alt-Shift-B?)
And the HL2DM that is sold on steam is actually the original HL2 source
engine, but the SDK code is up to date with the orange box engine, so the
SDK HL2DM is actually more up-to-date than the commercially sold version.

2) If your server shares the same internet ip as the computer that you are
using to attempt to connect to it (ie you connect to the internet through
the same internet connection on a network), then it won't show up.You need
to get a friend to see if it appears for you. If it doesn't
