I see this from time to time too. Restarting the server seems to "fix" the
problem about 50% of the time. There is no real pattern to it. One will go
unvac and then another will. Doesn't seem to matter what I do or configure.
I just have clients restart their servers a few times. It appears to me that
the servers don't get an answer from the vac auth (that's what I call it)
server in time and just time out and default to an insecure mode. Id much
rather have it default to secure than insecure but if the server cant talk
with the auth servers then it will do no good to be in secure mode anyway
since it cant get the list of baddies. How about a vac secure retry every x
if there is no -insecure switch? That might alleviate the problem.

This is a constant issue across our 20 CS and 20 CSS servers, we have
staggered the ports, staggered the restarts, but still they often and
irregularly will not go to secure mode.
I'm hoping our own dev team can add a few lines of code to our server
checker that well ensure the server stays at secure mode.

But in reality I would hope that VALVe could come up with a solution to
servers that wish to be secure will only start as such, after all we want
them secure, we don't run the insecure command, so we need VALVe in on this
as well, solving why they do not.
