When a server comes up if for some reason the Steam servers do not respond VAC mode is disabled. In the console you see the message, "Could not establish connection to Steam Server".
Then is says something similar to .. "VAC secure mode not activated". Then VAC is off for as long as the server is up. If you exit and restart the server a few times eventually it can get a response from the Steam servers and VAC is activated. You see the message ,"Connection to Steam servers Successful" and "VAC secure mode activated". The kicker is if your server resets during the day you have no idea when it comes up if it is in VAC mode or not. It gives the illusion of losing VAC.
