I've always run with sv_region 255. I do get players from the US, but it
seems they are in the miniority.

Switzerland
UK
Poland
Spain
Austria
Sweden
I don't really want to use 0 or 1, because if I do I loose half of the US
(is that correct?). Likewise with choosing any of the other settings. I'm
not complaining, mind you, I just find it curious.
If I look in the server browser, and set location to <All>, I get 13 hldms
servers, mine included. If I set it to US-East, I get the same 13. Any other
setting gives zero servers. Are there any kind souls that would like to try
this? Set Game to "Half-Life Deathmatch: Source". Clear all other settings.
Refresh. You should get about a dozen servers. Change Location to US-East,
refresh, then try US-West, refresh. What do you get?

I'm looking at my server, Half-Life DeathMatch Source, and not one
player
from is from the USA:
UK - 1
Turkey - 2
Russia - 3
Poland - 1
Austria - 1
This seems to be a fairly consistent pattern, I seem to get more
players
from Europe the the USA. I'm wondering if my lackluster ISP (Centel),
which
gives me less then stellar pings, is the reason why I don't get a lot
of
US
players? European players coming into a US server expect higher pings
and
tolerat it, US players want lower pings and go somewhere to find it?
Everyone is pinging in the 200 range right now.
