I used to have an issue similar to this on my dual 3GHz xeon box. The skipping mostly happened during large battles and map changes. Turns out it was logging - the scsi card and drives in this box aren't exactly the best, and the read/write overhead from all the disk writes due to logging *everything* was causing a significant amount of server lag under certain circumstances. So I turned logging to file off:
sv_logbans "1" sv_logecho "1" sv_logfile "0" While keeping logged events visible to the console window and to any other program (like hlstatsx or psychostats) that needed them. This solved my problem.
