The file is in /l4d_demo/left4dead/host.txt, it requires I believe a URL to load. I do not think it can accept straight HTML. The image size is 500x100px, but it is a bit larger for higher resolutions (1680x1050 for example). I'm not sure what the exact dimensions it renders at for high resolutions like that.
To keep it from looking bad in-game at high resolutions I just center the image and it shows up fine.
