I'd check for order of operations, and make sure that your server identity is set before it gets to the map command. Looking at that command line, for example, if you put it in server00.cfg, that probably would not be soon enough. You need to +exec the config file or set the identity on the command line, so it happens before the +map.

I tried a clean install without sourcemod. I don't know why my other servers with the identical settings all work and the 4 servers at this location will not login at all.
