This from Tony Ray, author of PunkBuster:
PunkBuster has support to not kick the HLTV proxy - it is currently
undocumented support. To allow HLTV to coexist with PB, get your HLTV proxy
working and then type status in your HLDS console to see what the "Player
Name" of the proxy is on your system ... for purposes of this example, I'll
assume it is "Proxy". Now, in your PB config file (default pbsvhl.cfg), add
the following line:
ProxyName proxy
This will cause the PB server to not kick any player named "proxy" as long
as that player never has any frags. I wish there was a better way to do it,
but for now, that should work.

I have verified this works. I had to use:
ProxyName "HLTV Proxy"
