I had this problem.

I have a athlon (model 2) with a GeForce and a SB Live! value.

It would happen the most when the system temp got too high and the Video
card would heat up.  If my computer room was too hot I could never play.  I
ended up disabling the agp 4x overclock to solve it.  Hasn't been an issue
since.

It took 6 months after building the system for it to become an issue.
Random freezes and such.  Then it would happen every time CS was done
connecting to a server.  Frustrated me to no end too.  Reinstall Windows,
upgraded to Win2k, tried with DX7 and DX8.  Now I can run openGL again.
