My server is CS. I will try to pin it down. Right now it seams to be random. I know it has happened on map change. But I have seen happen during gameplay too though I'm not sure if it happened at round end or while people were in motion.

It would seem to me that it happens at random times. While playing CS, I've gotten dropped during a map only once while in motion on de_rats. I sometimes get dropped when it changes. It happens every time (!) I don't have a map.
