To make it short, you see anyone with "%n" anywhere in their nickname, IP and steamid ban them permanently and as fast as your fingers can type - if they get an opportunity to die or suicide you'll crash out along with the rest of your server.
I would hate to see how insecure the rest of the code is if they haven't even put basic sanity checking on the usernames.
