Good stuff, a lot of information. I have a similar setup with 2 major exceptions, I don't have a caserver in the picture and the game servers are in a dmz style network with no nat. The clients however are natted in a similar method as yours.
I have 2 test servers up, 1 windows and 1 linux based both running cs:source as a dedicated non-listen servers. I also connect to the server with a different ip then I authenticate to the steam network with and I don't have this issue. Since it does work for some clients, I doubt there is any type of security in the way or network/firewall/router misconfigurations. If it will be any help, i'll be more than happy to offer a second set of eyes to look over the server configs to see if theres anything that stands out as different/incorrect.
