If you modify only the entities, the server/client does not consider it to
be a different map. The client will download the entity list from the server
map automatically. I wonder if it always does this regardless? IOW, will the
client always download the server map entity list even if they are the same?

Are you certain about this? Every instance I've ever seen where there is
a different .bsp file on the server and client, result in the user being
disconnected with a 'bsp files do not match' error.
