Have you added +ip x.x.x.x to the command line, where x.x.x.x is the public IP? We had a similar issue and adding that fixed it.

Sounds like a firewall issue. Check logs on your devices to see if anything is being dropped (incoming and outgoing.) By the sounds of it, probably look on your 'second router' first.
