My router never allowed me to connected via external IP before until I
forwarded port 1200 give that a try it might work for you.

This are the current ports being forwarded in my router.
TCP 27020-27039
UDP 1200
UDP 27000-27015

See if that resolves your issue.

        I think you explained it better than me.  However, people never have
a problem connecting to my server, only when they use the friends system and
attempt to connect to whatever server I am playing on (Join game) and that
server is my internal IP address server.

My NAT/Firewall is Linux iptables.  The ports are forwarded or no one could
connect and play.

I don't think the /etc/hosts idea do anything. The friends only has an IP
address.
