I have the same problem, and it only happens with hlds, and when I run the amxmodx mod with deathmatch plugin. It crashes as soon as someone attempts to join the server. 9 out of 10 times i see the assertion failed error when I start the server. It is very frustrating and happens on more than one server. My fix? keep starting/shutting down the server until the error disappears.
