Even sv_visiblemaxplayers won't always fix the issue.

If two clients join at the same time (say your server is 23/24 with one
reserve so technically 25 slots) they'll both hit the server. Whichever one
happens to have joined that millisecond earlier will be admitted to the
server, while the other client will be kicked for a reserve slot.

None of my servers have reserve slots. All of our clients (even our admins)
wait like everyone else, and if there's an issue we have SourceTV for
watching players rather than actually joining the server itself.
