I only run Sourcemod/Metamod and had just started using the Sourceop plugin to prevent the stringtable-related crashes in goldrush thanks to last week's update. But it occurs without any of these loaded as well. The user is not actually disconnecting, at least for us. They are still connected to the server. We don't see the disconnect by user message in the logs until the client disconencts from their menu after seeing the timeout in top right.
