Had this on l4d(1), only a handfull, did a full restart and all servers
full. Happens now and then..

On l4d2 i have not seen this that "often"  but less then l4d(1)
I do however see sometimes in netconsole a fork that has no current map
when i check with status. Most of those times the cpu is 100% for that
single fork too then.
