Interesting. So when player leaves from the server, the server doesn't know he/she is still there (not shown it status reply) but might aswell be (shown it server query) untill he/she shutsdown their Steam client (so Steam doesn't think the player is on the server even though he/she has already left). Perhaps this is why ghosts leave sometimes from the server.

So it's steamcommunity/server link related bug and perhaps thats why it's so hard to recreate in order to fix. Get on it, V.

Today when i wake up to go to work i noticed i have two ghosts on my
tf2
server. I have no source tv. No replay. Server.cfg is almost default.
How
can i get rid of them, without restart. My server is almost always
full.
