Got the same error this morning on our servers. There has been cases
like this while back time to time. Before it said something about
reverting back to fallback or something like that but no longer.

I've been getting Failed to update item schema on all my servers after a
recent update and restart. Probably isn't a big problem, but it is failing
on all my boxes.
