You need to include the old form when extending otherwise hobo has no
idea what you mean by <contact:> (btw it should be contact-tag)

<extend tag="form" for="Sale">
  <old-form merge>
    <contact-tag:><name-one /> ...

You might want to read through the dryml guide
http://cookbook.hobocentral.net/manual/dryml-guide.  it helped me get
a handle on parameters and attributes.
