Maybe this is too easy, but I think <a with="&some_object" to="&transition_name"> should give you a link to the transition page.
Instead of the transition-button tag, I often use the tag that's auto generated in forms.dryml. I'm fairly sure I've used them for create steps too. If you're still having problems I can look into it further.
