Unless your business model is a good fit for the cart I have found that it is usually easier to build one from scratch rather than try to modify someone elses code. A lot of those carts, at least the cheap ones that I have messed with, are either poorly commented procedural code or badly done components with hard to follow logic or Fusebox based. I am no fan of Fusebox.
All of the commercially available carts should have the Paypal connection already set up. You just select your merchant account provider in the cart administration section. I personally prefer Authorize.net. The last time I looked their rates were better and their technical support is superior to the rather poor support I have experienced from Paypal.
