If the IIS SMTP server IP address is set to "127.0.0.1" or  "All Unassigned", 
then that's what your localhost is using.

Right, but we have no other smtp service installed on that machine, so does

It means it is pointing to an SMTP relay/service on that web server.  It might 
just be the one that comes with IIS or something else installed on the server.
