I use Profiler and Tuning Advisor about once a month to check for
bottlenecks.  The db is pretty healthy, but we have certain queries that
cause issues just because of how they are doing wildcard searches of lots of
text.

Not a direct answer to your question...but...have you used SQL Server
Profiler and then Database Engine Tuning Advisor?  I'd be sure to do that
first before rewriting SQL...

quite a bit of   where field LIKE '%searchTerm%'.    Indexing can only get
so much gain and re-architecting the data is not an option right now.
