You can create other print queues just not the fax queue? Have you tried restarting cups and turning off the security features in cups?
Also try changing the cups loglevel to debug in the /etc/cups/cupsd.conf. Then open the log "tail -f /var/log/cups/error_log" and try readding the queue and post any errors in the log. Also do the same with the syslog. (tail -f /var/log/messages and then try readding the queue.)
