No, there's no reason usually to start with a clean database. If you're running htmerge/htpurge (as appropriate for your version of ht://Dig), this will clean out files. Granted, the database backend itself gets a bit less efficient if you're constantly adding and deleting lots of URLs (sorta like disk fragementation). But I don't think it's a big problem for most people.
