According to Tony Crockford:
> <select name="restrict" size="1">
> <option>in all member sites</option>
> <option value="namac.org">in NAMAC's site</option>
> </select>
>
> add value ="" to
>
>
> <option *Here* >in all member sites</option>
>
> and it will work.

The first search form also has a

                <input type="HIDDEN" name="exclude">

tag that's missing a value="" in it.

See http://www.htdig.org/FAQ.html#q5.17

Check all the hidden input tags and option tags in all your search forms,
both initial and followup (in your template files), to make sure there
aren't any others missing a value parameter. Also, when I tried a search
on your site, I consistently got...

   Not Found

The requested URL /namac/global.css was not found on this server.

So you're missing a style sheet file as well, or using a relative path
instead of a full path in the URL for it. Again, check your templates.
