For me, the key bit of information is that you aren't seeing the 3rd URL indexed at all. I'd check the config file and make sure either the line doesn't wrap, or that there's a line-continuation character '\' at the end of the line *and* that there's nothing after the '\' in the line before that 3rd URL.
If it doesn't index the 3rd URL, it certainly won't index the URLs in it.
