This is of course not the behavior most of us see. If I run with -vvv as you did, the output I see is almost identical to what you listed, aside from some differences in white-space. Is the page in question accessible to the outside world? A good first step would be to see if the problem can be reproduced by anyone else. Are you using your own build of ht://Dig or a prebuilt package (e.g. distributed with your OS)?
