Judging by the code, I see 3 possibilities:
1) You've set limit_normalized to something that excludes this normalized URL.
2) The current hop count reached max_hop_count.
3) htdig mistakenly thinks this URL has been visited (a bug in Dictionary.cc
on your system?)
