we did find that there were some files which weren't being re-indexed 
appropriately, which is a problem because our website is under constant
development by a team of about 10 people (one each for the major branches).
This was alleviated by removing the index files at a quiet time of day and
then regenerating them.

I haven't been able to go through either the code for 3.08b2 or the
patches due to time constraints, so I haven't been able to track the issue.
