Tested on 2016.0.0~rc2+sfsg-1ubuntu1~trusty Xubuntu 14.04LTS following the process in comment #1, the images now align and render well, however they end up in the nadir (using the Assistant / Simple Interface).

This occurs when the image is a portrait fisheye image. The focal length is out by the aspect ratio.
