I seem to remember that Andy Gill had a fairly reliable example, but
this was a long while ago (probably back in 93 or so).  It was a 3D
rendering algorithm that printed a perspective cube in ASCII
characters: quite often one of the sides of the cube would disappear.
Mark: do you still have a copy of this?  I can try to track it down if
not.
