Type 42-21 is the right answer, but to perhaps answer some of the questions raised in this pervasive thread about SMF 14 and 15's:
The MEMBER name is only populated when the member name is provided in the JCL, which causes the access to be BSAM or QSAM. When the access to the dataset is BPAM, there never will be a member name in the 14/15 records, and BPAM is used when you dynamically allocate a PDS library.
