The session appears to be terminated, but it would only appear to the other host. It is possible (likely?) that the session was actually terminated by a firewall posing as the other host.

Have you used "sshd -d" to get the server's debug options? As an aside, when debugging there's never too much information, so I'd use -vvv with the client for debugging.
