Sometimes it depends on how the pack will be used.  We have some SMS
managed packs (3390-2/-3) that end up with many small data sets.  For
those packs we often need more than two cylinders for the vtoc.

I cannot recall a case where the index needed to be bigger than the
VTOC.

  Is there a rule of thumb that I can use to code the parms when
initializing a volume via ICKDSF.  I am not sure what I should use when

   3380 model K  
   3390 model 2  
   3390 model 3  
   3390 model 9
  Below is what I am using to init a 3390-3 for example:
