The KSDS doesn't even have to be that "old" or "ancient". We had the same issue with KSDSs that were allocated under OS/390 V2.10. The datasets in question were allowed to use the default CI size for the index component as calculated by VSAM. After we migrated to z/OS 1.4, the calculation changed, so the CI size of the index component changed. When we tried to use dataset level FlashCopy, we also received to RC=20 error message.
Performing a DELETE/DEFINE of the dataset on z/OS 1.4 resolved the problem.
