I would agree. Get off that pack ASAP. I saw something like that when some ICKDSF function was performed from one LPAR while being online to another. Varying the unit offline to *all* LPARs and then back online seemed to fix the problem for us.
Another possibility is that volume is not protected by GRS or equivalent. Corruption (to include a VTOC) is to be expected. Yet another is a DFDSS volume restore while being online to any LPAR.
