There is an IOS data structure called the VOID (Vector Of IO Drivers) which, in my opinion anyway, is the definitive reference. Unfortunately, it looks to be OCO now.
At one time, I had a desire to use a SSCH Exit, whose address is defined in the VOID entry for each driver. I had asked IBM to consider a new service to dynamically define an I/O Driver (so that I could define my own SSCH Exit) but there was no other demand for it at the time. I seem to remember someone bringing up the subject again more recently, but I'm not sure if anything was ever actually done. The other nice thing about such a service would be the assignment of a unique(?) driver ID, rather than having several drivers in the system all running under the "Miscellaneous" driver ID.
