We have been experiencing an increased failure rate of the leader pins on our 
3592 media, and I was wondering if anyone else was seeing this.  We only have a 
few hundred of these tapes, but around 10% are currently out of circulation due 
to this problem.  There is a repair kit for this problem, but it only has parts 
to repair 10 tapes, and we have been through several kits to date.  At a cost 
of $100 or so, this is a repair cost of over $10/cartridge, which might be OK 
if it were rare, but this seems a little too much for me.
 
We do not have the 3592 drives in an ATL, and I am curious whether the human 
factor may be to blame.
 
Has anyone else on the list seen a significant failure rate on these 
cartridges?  Is there any known cause for this?  Any way to prevent the 
problem?  Has anyone tried to take warranty action on this?
