Unfortunately every plan is different for each site. There are a lot of commonalities and as was pointed out, they are mostly in the migration guides. I have a short checklist of 2400+/- items that I normally go through for a site when I'm going to convert them. Many of them are "don't forget to look at....", and with each new site I sometimes add new items. My list is on my PDA with links to manuals and other documents, sample jobs, recovery jobs, etc. which are also on the PDA (for when I forget). :)
