I guess there is no generally accepted philosophy.

I can remember sitting in on a JES2 presentation at Share by one of the
developers.
He asserted that the fixes from his team were so important (and clean)
they should be applied directly to all environments when shipped.
I almost fell off my chair - and I wasn't the only one.
