Ed, Such is the life of diminished mainframe work force at A&M, and a Managed Services company that mis-calculates how long it takes to get an OS up.

We experienced the problem in mid-2016 running z/OS 2.2 JES3 (which is no longer supported) and opened a PMR with IBM at that time. They took an APAR right away and we fix-tested it for them in the August/September time-frame IIRC. Is it possible you're still running z/OS 2.2 without having applied preventive service for nearly five years? If so, this could be your issue.
