Handled by the format-8, although the format-1 names are used for format-8
fields.

That seems to imply that at least one format-3 "n" exists already, and an
additional format-3 ("n+1") is to be added, so perhaps you mean "after the
first 16" instead of "after the first 3". Assuming at least one format-3 "n"
already exists, the CCHHR for the new "n+1" format-3 would be placed in the
"n+1" DS9F3 slot (unless all 10 slots are taken). The CCHHR of the first
format-3 is in the first DS9F3 slot and in DS9PTRDS.
I can't confirm that the DS3PTRDS of the first or "n" format-3 points to the
second or "n+1" format-3,
but it seems likely since that's the way format-3 DSCBs work for non-EAV data
sets with more than 16 extents.

Everything I haven't comment on looks correct to me.
