Not me. I always change both passwords and userids. Even for IBM (SERVICE user).
I also keep pre-defined users with no access from via web (only local).
I could delete pre-defined users, but I keep them "just for..."

To be honest, people are very reluctant, I heard many complaints.
Oh, no shared userids. Everyone has its own. Sometimes more than one (technical reasons), but still not shared.



Regarding password safe applications:
The most easy to use is just MS Word with encrypted document. :-)
The advantage is you can keep there anything, including looooong descriptions, pictures and even other files. The second can be Veracrypt encrypted filesystem (can be kept as regular file and mapped during open).

However if you want really secure password safe then use product which is *not* connected to the cloud or Internet. And *don't use it on your laptop*. It can be another device, even old (unconnected) smartphone. Air gap. Disadvantages: userids and passwords need to be manually retyped, no access "from the cloud", so if you forgot your device, you have no access to passwords. You have to maintain backup copies.
