I'll correct myself and apologize here.  My complaint about PDF
incompleteness was in error.  The PDF was in two-up format
and I failed to read the second column.  Otherwise, read on.

And the lockout occurs even if  I Cancel rather than Send!?
IBM isn't very good at UI design.  They may have used a contractor.

I bypass it by deleting cookies.
