So, if you provide more details it will help.

Is there an SVC Dump?  If not, set a slip trap.

Are there any other abends after the IEC999I?  COD, C03 or other?
Can you tell if your storage for the DCB may have been freed by the process
before closing?

Can you show us the Code for the DCB having the issue?

Are you using subroutines to handle the open/close/read of this DCB?  Are
you perhaps falling out of your process and ending before closing?
