My issue the files are all SMS managed and I am z/OS v1.12.  We have a lot of 
old tape media and I suspect some of my tape failures with HSM ML2 tape maybe 
part of the cause.

Do you have any issues with the tape media?  Are you pure virtual or Physical 
Tape for ML2?

What version of z/OS are you running?

Scenario is quite simple. Datasets have been migrated to ML2 tape. They expired 
so DFHSM marked the tape files deleted yet the catalogue entries still exit. 
For some reason DFHSM is not uncataloging the datasets from master catalogue 
during any space management cycles </snip>
